Explanation
HRT (hormone replacement therapy) is a form of hormonal therapy. In this context, it is used to refer to gender-affirming hormone therapy often pursued by transgender folks who want their body to match their gender. Due to the complexity of healthcare and widespread transphobia, it can be difficult to acquire through the medical route (talking with a doctor, etc.), leading many people to choose DIY options instead. Additionally, healthcare in countries like the USA can be very expensive for even the smallest consultations.
In the comic, the doctor is ignorant of the other person's request for HRT, and tries to walk around the subject, insults the person asking and hits them with a gouged-up bill despite not helping them.
